Breast Cancer

Purpose/Mission

To advocate for and improve quality care from screening through survivorship for people affected by Breast Cancer in the Colorado.

The Colorado Breast Cancer Task Force is a network of organizations and individuals that provides leadership and coordination of breast cancer services. It serves as a catalyst for breast cancer prevention and control activities throughout Colorado.

The Women's Wellness Connection (WWC) formerly the Colorado Women's Cancer Control Initiative has launched a new hotline number for women seeking the free WWC breast and cervical cancer screening.

The Toll-Free number is 1-866-951-WELL (9355).

"Provider Education Resources and Instructions for Presenting
"

Background and Information Regarding Reimbursement: The Provider Education Subcommittee received a small grant from the Denver Affiliate of Susan G. Komen for the Cure. With this grant we have a limited amount of funding available to provide refreshments to the audiences we are presenting to as well as offer mileage reimbursement to presenters. Due to this funding being limited, it is important that I know who is presenting where and when.
Therefore, if you’ll be presenting and seeking reimbursement for refreshments or mileage, please let me so that I can track this information and make sure there is funding available as needed. Also, this funding is only able to be utilized when presenting within the Denver Affiliate’s service area which includes the following counties: Adams, Arapahoe, Boulder, Broomfield, Clear Creek, Denver, Douglas, Gilpin, Jefferson, Larimer, Park and Summit Counties.

Making Modifications to PowerPoint Presentations:
I am interested in having any different variations of the PowerPoint presentations, based on any modifications that anyone makes. Please email me a copy if you make any revisions to the attached PowerPoint presentations. With that being said, when several of us presented at the Boulder Community Hospital we found that a lot of questions came up regarding the CWCCI contracted sites and Komen grantees in Boulder County, the county we were presenting it. Resources and Tools to Provide Training: Attached:List of targeted sites and presenters. If your name is attached to a targeted site above I’m assuming that you are planning on contacting the targeted provider. However if your name, or the site you are interested in presenting at is not listed in the targeted sites spreadsheet please let me know that you’ll be contacting a site BEFORE you contact them. This will hopefully insure that multiple people aren’t contacting the same site. Script to use in scheduling provider education trainings. Please keep in mind that our intended audience for these trainings is the scheduling or front desk staff. Our hope it to educate the staff that women attempting to schedule mammography appointments would be in touch with first. This will ensure that we are catching women and directing them to the services they may be eligible for as soon as possible.

Core PowerPoint Presentation – includes all of the general content of the presentation. Provider Specific PowerPoint Presentation – includes more specific slides re: screening guidelines, diagnostic services covered through CWCCI, how to become a CWCCI provider and the types of grant the Denver Affiliate funds.
Evaluation Survey – for the audience to complete, post hearing the presentation. Compile results and share findings to me via the “Provider Education Presentation Report”. Provider Education Presentation Report – Please complete, after presenting and return to me. Expense Reimbursement Form – Complete and submit to me, via email, fax or mail within 10 days of presenting. If I don’t receive this reimbursement form within 10 days of your presentation I will not be able to ensure that you’ll be reimbursed – due to the limited amount of funding and attempting to manage these funds effectively.

Available Hard Documents:
Provider Education Flow Chart and Fact Sheet – These have been printed and are available for distribution during training sessions. Please let me know if you’ll be presenting and I’ll make sure you get these materials. Breast Cancer Resource Directories – There are still copies of these directories available in both English and Spanish for distribution. If interested in taking copies of these with you, while presenting again let me know and I’ll make sure you get these materials. Again, thank you, thank you, thank you to everyone that has helped create these tools, determine targeted sites, and offered to present.
